Dr. Pui-yee Chan
Singapore Heart Foundation, Singapore

Pui Yee has worked in the various fields of physiotherapy, from caring for intensive care patients to outpatients with musculoskeletal problems, in various institutions over 20 years. Her work experience includes research, when she worked as a Research Assistant with the National University of Singapore on a project on the effects of load carrying on the posture of normal and scoliotic adolescents from 2002 to 2004, and when she joined the Rehabilitation Research Institute of Singapore, Nanyang Technological University, as a Research Associate from 2018 to 2020, where she was part of the team establishing an Asian-centric movement ability database. From 2015 to 2018, she was a lecturer at Republic Polytechnic, School of Health and Leisure, teaching various modules such as Sports Biomechanics, Health Psychology, Exercise Programming and Prescription. Currently, she is leading the physiotherapy team at the Singapore Heart Foundation, serving clients with / at-risk of cardiovascular diseases.

Abstract

Community Cardiac Rehab Programme at the Singapore Heart Foundation

Singapore Heart Foundation has been running its Phase 3 & 4 cardiac rehabilitation programme in the community setting for over a decade. The programme mainly comprises regular group exercise sessions and individual dietary counselling. Clients have to meet certain criteria to be admitted into the programme. Other measures are also put in place to ensure safety. Individual assessments and education sessions are conducted every 6-12 months for every client. Health and education talks, contests, cooking demonstrations and other group activities are regularly organised for the clients to support them in maintaining a healthy lifestyle.
